Advanced Directives


An advance directive is a legal document that will authorize the person designated to serve as your spokesperson if you are unable to make your own medical decisions.

If you already have an Advance Directive, we ask that you bring that with you when you come in for your preoperative testing or on the day of admission. If you do not already have an advance Directive, you may click here to download one in PDF format. The instructions on the form will guide you in completing the Advance Directive. Note: this actually describes medical power of attorney, which is just one form/aspect of an advance directive, as advance directives also include living wills even if no one is given power of attorney.
